Job description:

If you are a Registered Nurse looking for a flexible and rewarding role, we would love to hear from you. Home Instead is seeking an RN to support weekend virtual coverage across multiple office locations, with a focus on timely client assessments and care coordination.

This role is primarily virtual and plays a critical part in ensuring seamless weekend intake, assessment, and care set-up. There is also flexibility for additional weekday evening coverage based on operational need and interest.

The Client Care Manager is expected to perform a variety of duties including but not limited to:

• Determine appropriateness for virtual vs. in person assessments, conduct virtual care consultations
• Collaborate closely with weekend service coordinators and on-call teams to initiate services and ensure timely care set-up
• Develop initial care plans ensuring quality care and client satisfaction
• Oversee client, caregiver and/or PSW virtual introductions
• Maintain clear and concise documentation of all assessments and client communications
• Assist with overall problem solving, conflict resolution with staff or clients

Education

• Bachelor of Science in Nursing (or equivalent)

Qualifications

• Experience working in a Community Home Care setting
• Strong clinical assessment and critical thinking skills, with the ability to triage and make decisions independently
• Specialty experience with the geriatric population
• Comfortable conducting care assessments in a virtual environment when appropriate
• Excellent communication, organizational, problem solving and decision-making skills
• Ability to work independently and collaboratively within a team
• Demonstrated awareness of cultural diversity.

License/Certifications

• Current RN registration in good standing with The College of Nurses of Ontario
• Annual membership with The Registered Nurses Association of Ontario (including insurance coverage)
• Current First Aid & CPR certification

Experience

• Over 5-10 years experience in a general medical setting, retirement / long term care, and/or home care
• 2 -3 years of geriatric experience preferred

Schedule:

• Weekends (Saturday & Sunday): 12:00 PM – 8:00 PM
• Opportunity to pick up weekday evening shifts based on availability and operational need
• Provides coverage across multiple Home Instead offices

Pay: CA$35.00 - CA$40.00 per hour
